Due to a change in federal law, Colonial Staffing Group will no longer automatically mail 2024 Form 1095-Cs to employee homes. However, you may submit a written request to have your 2024 Form 1095-C provided to you. Request your Form through the following email: colleen@colonial-staffing.com , or send your request to:

Your form will be provided to you within thirty (30) days of the date your request is received, but in no event earlier than January 31, 2025.
